  • Home-based intervention to prevent functional decline in (pre-)frail older adults: the PromeTheus randomized controlled trial

Prof. Dr. med. Kilian Rapp, MPH is head of the ‘Abteilung für Geriatrie’ and of the scientific Department of Clinical Gerontology at Robert-Bosch-Hospital. He is specialised in Internal Medicine and Geriatrics, did postgraduate studies in “Public Health” and received a scientific education in epidemiology at the Institute of Epidemiology, Ulm University. He is a member of the European Academy for Medicine on Ageing, the German Society of Geriatric Medicine and the Fragility Fracture Network. His main research topics are falls prevention, fracture epidemiology and implementation research.

 

  • Referral to geriatric rehabilitation. Exploring triage

Aafke de Groot

From 1985 to 2020 Aafke de Groot worked as a physician for elderly care (community geriatrician) in the Netherlands. She founded the post-academic training course for geriatric rehabilitation experts (Gerion, Amsterdam VUMC) and is a lecturer at Amsterdam University Medical Centers.
In February 2026 she finished her PhD on triage.
In the thesis, we reported qualitative and quantitative research exploring the scientific foundation of triage practice.
We defined triage as a ‘professional assessment of a patients’ eligibility for geriatric rehabilitation’. In this assessment patient characteristics such as rehabilitation goals, rehabilitation potential and patient motivation/preferences are reviewed. Triage precedes an admission to post-acute care trajectory.
The lecture will provide a short overview of the studies that resulted in a conceptual model of triage with recommendations concerning the triage assessment, professional competencies concerning triage and organizational features that facilitate the quality of triage.
